Registered Nurse (Home Based Primary Care)

The HBPC Nurse Case manager is accountable for the comprehensive oversight of HBPC Nursing staff, delivery of patient care and the quality outcomes. He/she actively promotes the highest standards of nursing care and assures that nursing practice is based on best available evidence. The individual influences staff to meet organizational goals, mission vision and values. The HBPC Nurse Case Manager effectively communicates with the HBPC staff to ensure staff are familiar with policies/procedures. The HBPC Nurse Case Manager effectively collaborates with the HBPC Supervisors and HBPC Director regarding clinical and performance improvement activities. Functions or Scope of Assigned Duties:
include but not limited to:
a. Makes independent home visits to veterans with complex medical and psychosocial conditions. b. Frequent driving up to distances of 50 miles c. Case Managers manage caseloads of approximately 80-100 medical and psychosocially complex patients. d. Float RN's cover designated territories assisting assigned teams with home visits and covering Case Managers during periods of absence. e. Performs initial assessments and follow up visits for veterans referred to HBPC and uses the nursing process and evidence-based practice to develop an individualized, patient-driven holistic plan of care for each patient. f. Collaborates with the HBPC Interdisciplinary Team which includes the physician preceptor, RN, LPN, OT/PT, social worker, dietician, psychologist, PharmD and NP. g. Collaborates with services internal and external to the VA to manage the care needs of the patients. h. Provide patient and family health education with a focus on self-management, prevention, health promotion and wellness based on the patient's individual goals. i. Demonstrate leadership by serving as a preceptor or mentor to colleagues or students to enhance professional growth, development and practice. j. Completes all mandatory Talent Management System (TMS) training and CPR training by annual due date. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Experience:
Two years of successful nursing practice in the field of home health care or two years of inpatient medical experience Case management experience Bachelor of Science in Nursing Excellent interpersonal relationship skills Ability to work in an interdisciplinary setting Ability to work independently and to make competent clinical decisions using critical thinking skills Sound documentation, patient assessment skills, and interdisciplinary care planning Ability to be independent in the community Physical Requirements:
This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, pushing, lifting, moving, and transporting patients. Transferring patients and objects is required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or procedures. The incumbent may be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. In the patient clinical areas, hazards include risk of exposure to communicable diseases and risk of physical injury associated with the patient losing control. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations and able to shift priorities based on patient needs.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
